Some empty returns promoted to headers

Bakker Ch 1: see #64 for source file and outputs

These 5 headings are of the same level in the original doc. They all get promoted to an h1 (except for the 2nd one, but that's because of the trailing tab in #62):

  • Keeping the monies flowing the times of crises
  • The heroic migrant and the end of migration
  • The limits of migrant inclusion
  • Migration, state-led transnationalism, and development
  • The Washington Consensus and beyond: the continuing significance of market fundamentalism in development policy and practice

But, before the first 3 of these headings, an empty return gets promoted to an h4. The only difference I can see between the first 3 headings and the last 2 are in Word styles: the first 3 empty returns and the headings following them are class="Default" while the last 2 headings, where there's no erroneous empty header promotion, are class="FreeForm". Do you think the Word styles are the culprit behind this difference here?

The empty headings will get scrubbed out by the step that removes empty headers (#55 (closed)), but it would also be good to address the underlying cause of this empty return promotion as well.
