Note callouts appear twice in html

In the entire book by Boyles, the note callouts appear in the HTML two times for every note. One is the html note callout linked to the bottom, and the other is the plain text of the numeral that appears in the word doc:

I've used the first chapter as the attached example. In this chapter, the author's notes actually start at 2, which is why there's a difference of 1 between the number inserted during conversion and the number that comes through as text. The note numbers from the other chapters do match, but I thought using the mismatch example might be a bit easier for forensics.
Can you tell what it is about the author's implementation of the notes that is causing both the linked note callout and a plain text one to appear?
