Page numbering issues

There are a few issues related to page numbers:

1. The page numbers listed on the TOC for part pages are incorrect. Currently they list the blank page before the part page.
2. There's a similar numbering issue for chapters. The page numbers on the TOC either list:
   • the blank page (verso) before the chapter title page.
   • the last page (recto) of the previous chapter.
3. The last page of each chapter is missing a page number and running footer.
