Create Review and Decision forms using the Form builder
Description; system administrators need to be able to create a customisable 1) Review and 2) Decision form for completion by reviewers and editors during a peer review cycle.
Current state;
- Reviewers can capture 1) author feedback in a single textbox 2) editor feedback in a single textbox and 3) a recommendation; Accept, Revise or Reject.
- Editors can capture and submit feedback (share with the Author)in a single textbox in conjunction with a Decsion; Accept, Revise or Reject.
As per current functionality supported by the Form builder, Admins should be able to;
- Create a review/Decision form
- Add, remove and reorder form fields
- Apply advanced settings
- Submit a Decision/Recommendation
Acceptance criteria;

Admins should be able to select from 'Submission', 'Review' or 'Decision' from the Form dropdown menu. -
Admins should be able to create a Review/Decision form and select a form version as a default (currently enabled using Form purpose identifier=submit) -
Admins should be able to add, move and remove form fields. -
From builder field types, title, internal name, placeholder text and advanced settings should leverage existing Form builder functionality and UI.
