Review thread disappears when an author makes requested changes

To reproduce in a book requiring previewer(s) approval:

  • Author asks @Previewers (there were two in our test) to review and approve their preview
  • Previewer1 one requests Author to make changes
  • Author makes changes and reconverts / loads to preview
  • While author changes are converting Previewer2 comes in as requested to review the preview - THERE IS NO PREVIEW TO REVIEW WITH NO MESSAGE NOR IS THE REVIEW PANEL AVAILABLE TO TRACE THE COMMENTS TO FIGURE OUT WHY THERE IS NO PREVIEW TO REVIEW

This will make additional reviewers go crazy, we think. Particularly if they are clicking on preview links in email notifications (as we think many will do).
