TOC usability issues - some that will introduce content errors on Bookshelf

In our testing where Bookshelf staff is testing the PDF workflow support and the TOC support, we have run into the following usability issues some of which will introduce content errors on Bookshelf if not addressed in Phase 1:
- In submitting chapter-processed PDFs for tagging user was extremely confused how to submit even when provided documented instructions and seeing a demo. They wanted to use add part to create a place to submit their new chapter not seeing and understanding the Bulk Upload button. They actually did this and we have no way to delete it that will cause content TOC errors on Bookshelf. They also wanted to add the title for the chapter so they could track it on the page as we won't control or understand file names in the future.
See comments on the screenshot. To avoid content errors in Phase 1, we need to at minimum be able to delete parts per the red comment.
- As discussed there is still a lot of confusion about the TOC? icons, when we should see them, and their relation to labels - see comments / suggestions, the red at minimum needing resolution
