Display main headings from chapter in TOC

This option is added to the dropdown 'Display headings' in the 'new book' and 'settings' modals.
- Chapter title and main section headings
When the user previews the TOC, he sees the headings listed (as in this example) and can expand/collapse the headings as shown in the TOC preview design
The main headings are read from <title></title>
in <sec></sec>
in the xml file, for example:
