Create .cit file to send to NCBI

Submissions with manually created citations should have a .cit file generated from the citation data, and uploaded and attached to the manuscript as file.type 'citation'. These will be sent to the NCBI.

• Add db columns and form fields for citation urls
• Add .cit file to list to send to NCBI
• Create .cit file when citation info entered manually
• Delete .cit file if citation selected from search, overwriting manual citation
• Separate dropping PMID from entering manual citation info (PMID can be kept with manual citation data)
• Bonus: auto insert submission title into activity page citation search
