Accessibility improvements and refactoring

• refactor header component to use simpler markup
• fix accessibility errors in dashboard book list
• change number of items per page in dashboard to 12, for better wrapping in a grid
• replace "book action" popup with our own popup from coko client (more accessible)
• create new page to choose how user initializes the book (blank book or import files)
