Review of Question Creation and Submission wireframe

We've outlined the author's tasks when creating a question as follows:
- Author creates new submission
- Author chooses a question type
- Author adds question content, answers (with feedback), and metadata
- Author agrees to terms and conditions for the question
- Author submits questions
Step 1 is shown on the author's Dashboard above with the 'Create new question' button. This opens a page as below (with no content yet).
Revised design
(Note: we're leaving out colours to focus on functionality. Colours will be applied according to accessibility specs later).
Questions/comments:
- From looking at the sample questions in #1 (closed), 'Question title' doesn't seem relevant -- is that correct? In this design we've shown a snippet of the question text instead. This would also show in the Dashboard view (as above) changing 'title' column to 'question'.
- Question and answers are displayed side-by-side. Authors can add elements (e.g. an image or table) to both as needed.
- Space for feedback is shown below each answer. Is this correct or should there only be one feedback field for all the answers?
- Since the metadata is extensive we've given it more space in its own tab. An alternative could be to have questions with answers on left and all metadata on right.
- Author must check the 'terms and conditions' box before he can submit a question.
