PDF template changes

This specifically addresses #1216 (closed) and #1218 (closed) – footnotes were appearing inline, now they appear at the bottom of the page. Also the competing interest and for correspondence fields are now coming through.

Dates should also be working correctly – though the dates that we're taking in are just strings of text, so they're going to be displayed however the editor puts them in, which may well be nonsensical.

Note that this is using the existing Elife template; a new template for Amnet is in the works, which will incorporate these changes, though these are small enough (and useful enough) that I'm going to MR them.
