Chapter-processed book metadata are not "sticking" in the UI after press save

Expected behaviour
When a user adds author or any type of notes via UI for chapter processed book or collection metadata, after they press save, it whould remain stably saved in the UI for downstream processing.

Author notes are not "sticking" in the UI after press save

- Go to new chapter-processed book
- Add note
- Save
- Close modal
- Reopen meta modal
- Previous note no longer appears
Also -
Looks like the notes that disappeared from modal for previous loads are no longer written book metadata in converted XML once a new note is added. In one iteration @jordandc says an empty <p/>
in author note caused loading errors - see toc__43_.xml
Other reported issues of this:
User @douglassue added a collaborative / organization author AND publication history dates. After they were saved, next day they were gone.

QA Steps
- Create a Word Complete book
- Go to the book metadata & fill the required fields, the date created and date updated (from date history), Add a collaborative author and a note.
- Save the metadata
- Refresh the page and open the metadata modal again, confirm that the added data on step 2 is there.
- Add another note, Save the modal and check it again after refresh.
