Develop reviewer form based on grant PI data

This can be in the same component as #27 (closed) or in a new component.

The grant PI information from grants selected through the GRIST API must be used to create a form where the submitter selects a reviewer. The options should be:

• Me (User name)
• Full names of grant PIs from selected grants (Separate options for each)
• Other (Form to input surname, given names, and email address)

The form should appear on the same page as the grant search but should be disabled until at least one grant is selected.
