NCBI testing release 3 Feb: covers support

Feature: cover support
The main feature in this release is covers support.
- As a System Admin or Org Admin, upload a cover to a collection which can optionally apply to all the books in the collection. This feature is documented in #355 (closed).
- As a System Admin or Org Admin or Editor, upload a cover to a chapter-processed book. This feature is documented in #423 (closed)
- As a System Admin or Org Admin or Editor, upload a cover to a wholebook. This feature is documented in #819 (closed)
- As an FTP submitter (no BCMS role required), submit a cover in wholebook, package according to the submission specs (#183), and see it applied in the UI.
- As an PDF2XML vendor, submit a cover in the conversion package, according to the submission specs in #404 (closed), and see it applied in the UI.
- When a user uploads a cover in the UI or submits a cover in an FTP package, the cover is sent to NCBI systems according to the specifications in #751 (closed)
- When a user uploads a cover in the UI or submits a cover in an FTP package that cannot be displayed due to its file type, display a default message so the users knows the upload was successful. This feature is documented in #646 (closed). Note this was implemented before, but needed a change because of the new meta forms.
